To ensure the quality of the care we provide, Dr. White does not contract with or participate in any dental plans or insurance. Dr. White is not an in-network provider. This doesn't mean that we do not accept your insurance or that your insurance company will not pay any benefits. It may not make a difference, or they may pay at a reduced rate. We suggest you contact your insurance company to get a benefit summary prior to treatment.
As a courtesy to our patients, we are happy to verify your benefits, file your claims and estimate your costs (as closely as possible). However, your portion is due at time of service. The patient/guarantor is responsible for any amount not covered by insurance.

As a condition of your treatment by our practice, we request that financial arrangements be made in advance. Our practice depends on reimbursement from the patients for their costs incurred for their care. Therefore, financial responsibility must be determined for the patient and our practice before treatment begins.
All emergency patients are expected to pay for services rendered unless financial arrangements have been made prior to treatment. However, we cannot diagnose and treat over the phone.
